Please submit copies of vaccination cards (if you have not already done so) for all attendees ages 5+ to Reform Temple of Forest Hills to services@rtfh.org.  If you do not submit vaccination cards in advance, you can either bring them to the RTFH office before the High Holy Days or you must bring them with you to services.  Once you show proof of vaccination, you will then be able to pick up your tickets.

Masks are optional but encouraged.
